Position Overview
As a Physical Therapist, you will evaluate residents, create individualized treatment plans, and implement therapy services to restore mobility, relieve pain, and promote functional independence. You will collaborate with interdisciplinary teams and maintain documentation in compliance with applicable regulations.
Key Responsibilities
- Evaluate patients’ physical conditions and functional limitations.
- Develop and implement individualized treatment plans using evidence-based practices.
- Administer physical therapy interventions to improve strength, balance, and mobility.
- Monitor patient progress and adjust treatment plans as needed.
- Educate residents, families, and caregivers on exercises and care techniques.
- Ensure accurate, timely documentation according to facility and regulatory standards.
- Attend interdisciplinary team meetings and contribute to patient care planning.
- Adhere to all safety protocols and regulatory requirements.
Qualifications
- Active Texas Physical Therapist license
- In good standing with all relevant licensing boards and agencies
- Experience in Skilled Nursing Facilities (SNF) or long-term care preferred
- Familiarity with Net Health or similar documentation system a plus
- Excellent interpersonal and communication skills
