The Healthcare Surveyor / Community Programs Healthcare Surveyor will:

  • Ensure individuals with intellectual and developmental disabilities (I/DD) receive safe quality services as required by their individualized service plans;
  • Verify that staff are properly screened and trained to meet individuals’ needs;
  • Assess service quality;
  • Ensure compliance with State and Federal requirements;
  • Analyze data;
  • Conduct on-site interviews;
  • Record reviews and observations;
  • Write formal reports of findings;

How does it get done?

The Healthcare Surveyor / Community Programs Healthcare Surveyor is responsible for:

  • Having knowledge of and/ or ability to learn applicable Federal and State standards and regulations affecting programs and agencies involved in providing waiver services;
  • Participating as a Team Lead or Team Member in assigned surveys, including required Statewide travel;
  • Routine compliance survey preparation, survey participation, and completing formal reports of findings;
  • Coordinating and communicating logistical information regarding the survey to team members, and providing leadership to the survey team through clear and concise information;
  • Possessing skills in gathering, interpreting and analyzing data and making informed decisions to determine compliance status;
  • Completing all required Health Care Authority (HCA), Division of Health Improvement (DHI) and Developmental Disabilities Supports Division (DDSD) trainings, including Quality Management Bureau (QMB) core surveying competency training;
  • Providing excellent customer service to internal and external customers of QMB and DHI.

  • Individuals receiving services, as well as their families, guardians, and advocates;
  • Developmental Disabilities Supports Division (DDSD);
  • Quality Management Bureau (QMB), which works in close coordination with other divisions and bureaus within the Health Care Authority (HCA);
  • Additional stakeholders;
  • The community at large.

Ideal Candidate

The Ideal Candidate has:

  • Experience with Medicaid Waivers (Developmental Disabilities, Mi Via, and Medically Fragile);
  • Experience supporting individuals with intellectual and developmental disabilities;
  • Strong verbal and written communication skills, including editing and proofreading with attention to detail;
  • Proficiency with standard computer systems and equipment;
  • Team-orientation with a willingness to learn and adapt;
  • Knowledge of applicable Federal and State standards and regulations affecting programs and agencies involved in providing waiver services;
  • Experience gathering data, interpreting and analyzing the data and making informed decisions based on evidence gathered;
  • Excellent customer service to internal and external customers;
  • Ability to travel as needed;

Minimum Qualification

Associate’s degree from an accredited college or university in Nursing, Architecture, Social Work, Sociology, Psychology, Rehabilitation, and/or Criminal Justice and two (2) years directly related experience in community-based healthcare services and case management. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling four (4) years may substitute for the required education and experience.

Employment Requirements

Must possess and maintain a valid Driver’s License.

Must obtain a Defensive Driving Certificate.

Working Conditions

Work is performed in an office setting and in the field at community program sites. Out-of-state travel for training, and extensive statewide, with the occasional overnight travel is required. On-call duties may be required.
